Please note that Dr. Pytiak will only see patients ages 7-18. He treats sports injuries (excluding feet, hips, and back). He will also treat osteochondritis syndrome. If you need to be seen for a fracture, please call the office directly at (303) 861-2663.

New patients can download and print the New Patient Paperwork by clicking here. Please be sure to bring the completed forms with you and arrive 30 minutes prior to your scheduled appointment. Please also be sure to bring any x-rays, MRIs, ultrasounds, or CT scans to the appointment as well. A parent or legal guardian must be present for the appointment.

All patients, please bring the following to expedite your registration process:

*I.D. or Driver’s License
*Patient’s Social Security Number
*Any applicable co-pay/balance

We require a referral for patients with the following insurance plans prior to being scheduled:
